Alessandro G. Littara (born 15 April 1964, Pisa) is an Italian surgeon specialising in male genital aesthetic surgery, particularly phalloplasty (penile lengthening and girth enhancement). He is the founder of the International Academy of Penoplasty and is widely cited as one of the world's most experienced surgeons in his field, with a documented case series of over 7,000 procedures.

Education

Littara graduated in Medicine with highest honours (summa cum laude) from the University of Pisa, where he also completed his specialisation in General Surgery at the Institute of General and Experimental Surgery (Ospedale Cisanello, Pisa). He holds further postgraduate qualifications in ultrasound diagnostics in surgery, andrology, and sexology, all from the University of Pisa.

He was selected by competitive examination to attend the European School of Andrology and Andrological Surgery, organised by the Italian Society of Andrology at leading European centres. He completed training in female genital laser surgery (LVR and DLV procedures) at the Laser Vaginal Rejuvenation Center in Beverly Hills, directed by Dr. David Matlock, and holds affiliate surgeon accreditation from that centre.

Career

Littara began his clinical activity in the early 1990s, training at the Andrology Centre of the University of Pisa under Professor G.F. Menchini Fabris. He subsequently developed his uro-andrological surgical expertise under Professors E. Belgrano (University of Trieste) and F. Carmignani (University of Genova) over a period of more than five years, and completed an update period at the IRCCS San Raffaele in Milan under Professor Rigatti.

He served as adjunct professor (Professore a contratto) of Reconstructive Uro-genital Surgery at the University of Pisa, and was head of the andrological and surgical sexology unit at the Mercurio-Quisisana clinic in Montecatini Terme, where he participated in approximately 1,000 andrological and uro-gynaecological procedures.

He currently practises at Blumar Medica in Milan and at Centro Chirurgico Madra in Florence, and operates periodically in Germany (UGRS International Services GmbH, Darmstadt) and in the United Kingdom (London).

Phalloplasty: technique and contributions

Littara is considered among the surgeons with the largest documented personal case series in male aesthetic phalloplasty worldwide, with over 7,000 procedures performed.[1] His research has focused on the development of an original technique that avoids the use of any foreign material, relying exclusively on autologous fat transfer (lipofilling) for both penile lengthening and girth enhancement.

His techniques have been cited as a clinical benchmark in the EAU Guidelines on Sexual and Reproductive Health 2026 (European Association of Urology), Chapter 9 (Penile Size Abnormalities), reference. The guidelines report outcomes from his series of 355 patients, documenting a statistically significant increase in penile length at rest from 8.8 cm to 11.4 cm, stretched penile length from 12.4 cm to 13.5 cm, and circumference at rest from 8.3 cm to 11.06 cm at 12-month follow-up, along with improvement in IIEF-5 scores.[2]

International Academy of Penoplasty

Littara founded the International Academy of Penoplasty (IAP) with the aim of training surgeons in penoplasty techniques and disseminating evidence-based standards internationally. Within the first three years of its operation, the Academy trained over 50 surgeons from countries around the world; the total now exceeds 60.[3]

Penisgate (2026 Winter Olympics)

In February 2026, during the Milan-Cortina Winter Olympics, Littara publicly stated that he had performed a hyaluronic acid penile injection on a ski jumping athlete approximately one month before the competition. The declaration fuelled an international controversy — widely referred to in English-language media as "Penisgate" — regarding the potential aerodynamic advantage that a modified penile profile might confer on a ski jumper during competition.

The story was covered by major international outlets across more than twenty countries, including USA Today,[5] New York Post,[6] Daily Mail,[7] Le Parisien,[8] Yahoo Sports,[9] Marca,[10] SVT,[11] Aftonbladet,[12] Expressen,[13] de Telegraaf,[14] RMC Sport/BFM TV,[15] Nettavisen,[16] Yle,[17] and Sapo/Portugal.[18]

Media presence

Throughout his career, Littara has appeared as a medical expert on Italian national television and radio programmes, including Grand Hotel Chiambretti (Canale 5, 2015), Lucignolo 2.0 (Italia 1, 2014), and the radio programme La Zanzara (Radio 24, multiple editions). He has contributed as an expert to Istruzioni per l'uso on Rai 1 and Rai 3.
